This Case Manager position ensures quality, integrated care is implemented for specific patient populations throughout their life span. This position is responsible for establishing trust between the patient and their care team. The Case Manager provides continuity of care for their empanelment through leadership and expertise in the delivery of not only chronic disease management but also preventative care. This position works in an outpatient setting to promote progressive and healthy patient outcomes through a holistic care approach. This is a Population-Based Case Management role utilizing both external and internal resources to meet the needs of every patient.
Baseline Qualification Requirements:
Graduate of an accredited school of nursing (Bachelor’s degree preferred).
Licensure as a Registered Nurse
2 year of general nursing experience with a minimum of 1 year in an ambulatory setting or of 1 year case management experience.
