Survivorship Nurse Navigator Salary Range: $5916.67 to $12733.33 Monthly Final Filing Date: 02/13/2019 Survivorship Nurse Navigator Responsibilities: The UC Davis Comprehensive Cancer Center provides medical and surgical oncology specialty and subspecialty care to cancer patients starting with diagnosis and extending through, treatment, surveillance and survivorship. A survivorship care plan is a comprehensive cancer treatment summary document that includes the treatment received during active cancer treatment, a summary of post-treatment care needs, and follow-up/surveillance care instructions to improve health and quality of life. The position of Survivorship Nurse Navigator is responsible for identifying eligible patients who should receive a survivorship care plan; developing survivorship care plans for eligible patients; ensuring the plans are reviewed and approved by the managing oncology providers/teams; collaborating with the providers/teams to establish processes for dissemination of the plans; and monitoring and tracking survivorship care plans delivered to patients. The Survivorship Nurse Navigator will collaborate with the supportive care program team, including other Navigators, to provide information useful in developing the program. This incumbent will be a technical leader with a high degree of knowledge and recognized expertise in oncology, cancer survivorship, and data abstraction/management. Survivorship navigation will include formulating strategies needed to develop policies, processes, and resources in coordination with the Cancer Center Clinic team and the UCD Cancer Committee. This position involves a high level of autonomy. The incumbent may provide survivorship education and a comprehensive review of the survivorship care plan to patients in conjunction with the clinical team of registered nurses, advanced practitioners, and physicians. Survivorship Nurse Navigator Qualifications: REQUIRED - At least 3 years of recent RN experience in oncology, survivorship, and/or data abstraction /management.
- Valid California RN license.
- Bachelor of Science in Nursing.
- Knowledge of clinical-based computer systems and software.
- Clinical knowledge in oncology and survivorship, including medical oncology, surgical oncology, and/or radiation oncology.
- Advanced knowledge in data abstraction and management.
- Prior experience with following established clinical abstraction guidelines and/or developing criteria for data abstraction
- Proven ability to initiate projects, prioritize tasks, and direct projects successfully from inception to completion on task and on schedule
- Experience in navigating data bases, data collection, tracking and reporting
- Advanced skills and expertise working in a complex, multi-divisional environment
- Advanced communication skills to collaborate with multi-disciplinary teams and teach patients about complex, sensitive clinical issues
- Ability to intervene and assist with patient's questions or concerns about a treatment plan
- Advanced skills with Windows-based software, including Microsoft Word, Excel, and Outlook
