Rockford Center is seeking a dynamic and talented Licensed Practical Nurse (LPN).
Rockford Center is Delaware's first private psychiatric facility and remains a leader in providing behavioral health care services and continues its commitment to providing quality health care. Rockford Center, a 138-bed acute psychiatric hospital continues to provide a full continuum of behavioral health services to geriatrics, adults, adolescents, and children.
The LPN will deliver quality nursing care to psychiatric adult, geriatric, adolescent and child patients.
Key Responsibilities include:
Conducts ongoing and individualized nursing care duties as assigned and as dictated by the patient's condition in accordance with physician orders and facility protocols.
Monitors for and reports change in patient physical or mental status.
Collaborates with other team members in the development, implementation, and evaluation of an individualized plan of care.
Administer patient medications as prescribed by physician.
Provide education to patients on their prescribed medications, documenting this in the patient record.
Documents shift observation of patients as assigned, following up on vital signs and accu-check results prior to dispensing medication.
Transcription of medication orders in a timely, error free manner, and ensuring reason for PRN medication is documented.
