M Health Fairview has an opening for an Advance Care Planning Liaison in the Honoring Choices Department supporting Advance Care Planning across the system including UMP and Ebenezer locations. Liaisons work collaboratively to establish and maintain Advance Care Planning resources and services for patients, residents, the community and staff. This position will have primary accountability for training and education in addition to supporting the team in maintaining system standards and protocols.
This is a full time (1.0 FTE, 40 Hours/Week), benefits - eligible opening. This position will include both remote and on-site responsibilities with local travel possible
Responsibilities: Duties and responsibilities include but are not limited to:
Promotion and presentation of ACP classes utilizing multiple platforms for patients, residents and the community
Promotion and presentation of ACP facilitator, provider & physician training utilizing multiple platforms.
Evaluate, ensure, and support compliance with regulatory requirements at all sites.
Assist with validation, entry, and documentation in the medical record for Advance Care Planning and Legal Decision Maker documents.
Assist with establishing, achieving, evaluating and reporting of team goals in alignment with M Health Fairview performance dimensions.
Education: BA or BS
One year demonstrated knowledge/experience with Advance Care Planning services
Demonstrated knowledge/experience in building relationships with system teams to achieve specified outcomes
Proficiency in Microsoft applications (including Outlook, Word, Excel, Power Point, Teams) and virtual meeting platforms
