Serves as a member of the Home Based Primary Care (HBPC) interdisciplinary team based at the St. Louis VA Health Care System. This Registered Nurse is responsible and accountable for the initial and continuous assessment for a culturally diverse veteran patient population in the home setting. In addition to assessment, this nurse evaluates, plans, implements, coordinates care and confers with Home Based Primary Care team in delivering care to veterans in the home care setting. The Home Based Primary Care Nurse possesses the knowledge and skills to effectively and collaboratively apply all aspects of the nursing process within the area of assignment. Role responsibilities and accountabilities also include demonstration of the knowledge and skills necessary to assess the care appropriate to the age related needs of patients served in the home setting. Duties include but are not limited to: Provides primary care at the Veteran's home as part of the Spinal Cord Injury (SCI) HBPC team. Provides teaching to the patient and caregiver in ways of managing and delivering nursing care in the home; monitors patient's condition and the care required in the home and collects blood and other specimens required for monitoring the effectiveness of interventions and/or disease progression. Provides needed assistance and consultation for home care referrals. The Home Based Primary Care Nurse possesses the knowledge and skills to effectively and collaboratively apply all aspects of the nursing process within the area of assignment. Role responsibilities and accountabilities also include demonstration of the knowledge and skills necessary to assess the care appropriate to the age related needs of patients served in the home setting. This knowledge includes understanding changes associated with aging and principles of growth and development relevant to the adult and geriatric population. 1. Demonstrates successful performance in applying the nursing process. 2. Participates in performance improvement activities in the Spinal Cord Injury service line and Medical Center. 3. Demonstrates knowledge and skills in the home care of the spinal cord injury, quadriplegic and paraplegic patient. 4. Demonstrates an excellent customer service philosophy with both internal and external customers. 5. Demonstrates collaborative relationships with the Inter-disciplinary teams in providing continuity of care to the patient. 6. Demonstrates self-motivation and independence in professional development, organization of tasks/assignments for the nursing team and in providing a safe patient care environment. 7. Demonstrates knowledge and skills in the completion of spinal cord injury care, documentation and standards (i.e. CARF guidelines for rehab, sustaining care program).
