| The Community Living Center (CLC) Licensed Practical Nurse (LPN), GS-7 is directly responsible to the nurse manager or assigned supervisor, and works independently, with readily available guidance from the registered nurse or provider. The (CLC) LPN, GS-7 is accountable for adhering to established policies, procedures, standards of care and standards of practice, and for maintaining competencies consistent with roles and responsibilities in the Community Living Center. The (CLC) LPN GS-7: Provides care in a safe and cost-effective manner. Contributes to the evaluation of fall risk residents and implements appropriate preventative measures. Incorporates current resident care data by actively contributing to the development and modification of care plans. Contributes to the evaluation of pain/comfort status consistently. Identifies and encourages participation in the rehabilitation aspect of resident care. Identifies and encourages participation in restorative care. Demonstrates knowledge of medical equipment and supplies with advanced skill. Demonstrates the ability to perform venipuncture procedures. Recognizes co-worker's need for assistance and responds accordingly. Serves as a primary preceptor for newly assigned LPN, NA staff, and students. Promptly reports all accidents and notifies supervisor of unsafe/unhealthful conditions in the workplace. Performs capillary glucose testing, evaluates glucose results, and acts accordingly. Demonstrates the knowledge, skill, ability, and respond appropriately to emergency situations and unpredictable behaviors. Demonstrates the ability to promote teamwork consistently and fosters cohesiveness among team members. Complies with all safety/fire prevention rules/regulations, using protective equipment when required and provided. Interprets mental and physical changes based on theoretical and practical understanding of psychology, anatomy, and physiology. Promptly informs the RN or provider of observed changes in resident condition and initiating appropriate, prescribed intervention with safe and therapeutic resident care delivery, within scope of practice. Assists residents to cope with and/or compensate for changes affecting mobility and balance for geriatric residents, residents with impaired mobility, surgery residents, and orthopedic residents. Assists the registered nurse (RN) and/or provider to appropriately prioritize overall resident care needs and adjust plans of care delivery. Demonstrates knowledge of the principles of resident teaching, effective communication techniques, conducting nursing groups, conference participation, and initiation. Communicates calmly, courteously, and tactfully with all residents, personnel, and visitors maintaining confidentiality in accordance with HIPAA regulations and medical center policies. Demonstrates broad knowledge and skill with respect to medications, contraindications, actions, and recognition of the more subtle signs and symptoms of potential actual drug reactions. Possesses the knowledge and skills required for procurement, set-up, and use and re-processing of reusable medical equipment (RME). Promotes the values of cultural transformation and Veteran Centered Care to residents and nursing staff in the Community Living Center. Functions independently as the Team Leader for a designated group of residents or assists with charge in the Community Living Center.
