Outpatient services are provided in a colorful patient setting in our Total Care Pavilion and there will soon be nurse navigators to assist orthopedic and cancer patients. Los Alamitos Medical Center is a great place to work. Provides social work services to patients and families to facilitate their ability to cope with the psychological and social problems related to illness. Provides abuse detection and reporting, information and referral, consultation and counseling and assistance with discharge planning. Participates in interdisciplinary care process, in-services and other education programs of the hospital. The position requires knowledge of developmental stages as service is provided to infant, child, adolescent, Pediatric and geriatric populations. This position also requires knowledge of cultural influences in response to illness as there is come cultural diversity in the population served.
Qualifications: Master's Degree is Social Work from an accredited University or Bachelor's Degree in Social Work from an accredited.
At least 1 year in an acute medical facility preferred, or related experience in a medical setting.
Excellent verbal, written and computer skills
Excellent organizational and communication skills
Knowledge of internal policies and State/Federal regulations related to discharge planning and social services
