He/she provides care coordination for veterans who are assigned to a Patient Aligned Care Team (PACT) for the Shavano Park Outpatient Clinic on the Northside of San Antonio. The PACT will be located at Shavano Park Outpatient Clinic (SPOPC). The incumbent may be required to staff at NCFC as patient care needs require. We are seeking a Registered Nurse who will provide primary care nursing support for a rapidly growing population. The RN will consistently be able to fulfill the standards of Nursing Practice in accordance with established policies and procedures of the Department of Veterans Affairs and Nursing Service. The RN will perform a wide range of clinical and/or administrative duties and will utilize nursing skills when assessing and triaging unscheduled patients as walk-ins, over the phone or via secure messaging. Assures current, comprehensive, and timely documentation is contained in the patient's medical record and any other pertinent records. Uses the nursing process, evidence-based practice, and effective communication to collaborate with healthcare providers, family and/or caregiver as well as internal VHA and community based services. The RN must work collaboratively within an intradisciplinary team to assist in the care of complex populations. Performs a variety of tasks to include but not limited to: Vital signs, EKGs, immunizations, injections, blood cultures, PICC line management, setting up and assisting with procedures, wound care and other tasks as required. The RN oversees the duties and performance of the LVNs. May function as charge nurse and preceptor; prepares schedule, makes/adjusts staffing assignments, etc. They will foster/support a learning environment by participation in the orientation of new employees, providing feedback as appropriate and seeking opportunities for self-improvement. The RN will also perform telehealth duties as required, which may include but not limited to: Assisting providers with set and assessment via telehealth equipment, conducting duties related to telemed and telederm. The RN promotes patient self-management, prevention, and wellness; while incorporating critical thinking and customer service skills into all aspects of the role. Must also be able to navigate the electronic health record, as well as databases/tools that support Ambulatory Sensitive Conditions and Disease Management. Must be able to participate in all PACT activities, huddles, and training as well as other relative training. Work Schedule: Monday-Friday, 8:00am - 4:30pm (Tour of Duty and location may vary depending on patient care needs and staffing requirements, to include weekend and extended hours.
