The Central Texas Veterans Health Care System is currently recruiting for a Nurse Practitioner (Primary Care) position located at the LaGrange Community Based Outpatient Clinic. You will practice according to the guidelines set forth under VA's APRN Full Practice Authority (FPA). You will function within the scope of practice commensurate with their training, demonstrated expertise, certification, and state licensure. Continue to perform many of the same interventions in basic nursing practice in addition to a greater degree of syntheses of data, and complexity of skills and interventions, and will exercise of a high degree of independent judgment. Demonstrate leadership in delivering and improving holistic care through collaborative strategies. Responsible and accountable to provide primary health care and/or specialty care across the spectrum of clinical settings. Your primary function is as a direct care provider and/or expert clinicians to include, but not limited to, education, research, consultation, hands on patient care, and administration. FPA is the dynamic and skilled application of a systematic body of knowledge to the diagnosis and treatment of actual or potential health problems (ANA 2004). You will consider all elements of the nursing process when providing and/or supervising direct patient care. Assess, plan, implement and evaluate care based the needs of the patient. Assume responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management before and after admission and discharge, and supporting factors to provide proactive, personalized and patient centered care that impact customer satisfaction. Consider all characteristics of the individual, including age and life stages, state of health, race and culture, values, and previous experiences. Responsible for collaborating with internal and external services to facilitate care transition in order to effectively meet the patients' needs and provides a robust interdisciplinary approach to care; health promotion or disease prevention, acute, chronic, and long-term needs, education, and coordinates discharge follow-up. Administer medications and procedures per established policies and guidelines. Influence care outcomes by collaborating with members of the interdisciplinary team. Core elements of performance include knowledge and active participation in unit/program level quality improvement processes and initiatives as well as customer service programs. You will serve as an advocate for patients and is clinically supportive to staff. You will manage clinically complex patients and situations to include patient care calls. Function independently, be self-directed in clinical practice and career development, and actively involved in the functions for Primary Care related issues. It is expected that you provide safe, effective, quality care to patients and possess the management skills necessary to function as a provider in a virtual environment. You will offer primary care in both a face to face environment in La Grange and virtual environment supporting primary care needs of the healthcare system. Other duties as assigned.
