These positions are located in the Primary Care - PACT units at the Northport VA Medical Center. Duties/Responsibilities include, but are not limited to: The RN uses the nursing process and evidence-based practice to collaborate with the Teamlet (Veteran, Primary Care Provider, RN and Medical Support Assistant) and larger Team (family/ caregiver, internal and community-based services involved in providing care to the Veteran) in developing the patient-centered holistic care plan for life. The RN is a member of the Teamlet team. The RN is responsible for collaborating with services internal and external to the VA to facilitate care transition in order to effectively meet the Veterans' needs. The RN provides patient and family health education with a focus on self-management, prevention, and wellness, based on the Veteran's goals. The RN demonstrates leadership by serving as an advocate for Veterans and as a team player to colleagues as she/he continues to enhance his/her own and the team's professional growth, development and practice. The Staff RN is also responsible and accountable for maintaining training required and may be assigned additional duties as area needs require. The Staff RN is expected to communicate timely and effectively with all members of the patient care team, as well as relate courteously and appropriately to all internal and external customers. The Staff RN is expected to work in a collaborative, team-oriented environment, and demonstrate consistent effective communication skills and exemplary professional behaviors. Fostering positive interdisciplinary collaborative relationships is essential for continuity of care that is safe and quality driven Work Schedule: Full time; Monday through Friday: 7:30 AM to 4:00 PM OR 8:00 AM to 4:30 PM (depending on assigned Clinic/needs of the service) Financial Disclosure Report: Not required
