The RN Coordinator is responsible for receiving referrals and consults for veterans to the Structural Heart program. The RN Coordinator maintains and tracks progress for referrals and consults as veterans are evaluated for a possible future Structural Heart procedure as screening and evaluation is completed. The RN Coordinator is essential in coordination of care for the veteran with a consult to the structural heart program. Major Duties include but are not limited to: Assures that all essential diagnostic tests are ordered and completed in a timely manner for each patient and extracting the relevant data to allow for accurate screening Take part in the specialized Structural Heart clinic and will organize and schedule the regular inter-disciplinary Structural Heart meetings, which serve to allow discussion of patient eligibility and procedural specifics Tracking patient outcomes for Quality Assurance purposes Primary point of contact for the Structural Heart procedure device vendor(s) and will work with cath lab and OR staff to assure that all necessary equipment is kept in appropriate stock to allow for safe performance of the procedures Planning and scheduling of procedures assuring the availability of the patient, members of the multi-disciplinary team and required devices. Assumes responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction Administers medications and procedures per established policies and guidelines Influences care outcomes by collaborating with members of the interdisciplinary team Core elements of performance include knowledge and active participation in unit/program level quality improvement processes and initiatives as well as customer service programs.
