The Licensed Practical Nurse (LPN) is administratively responsible and accountable to the Nurse Manager (NM)/designee. The LPN is accountable for delivering patient care to the adult/geriatric patient population with varying diagnoses and treatments and is an essential component of the Patient Aligned Care Team initiative (PACT). The LPN is assigned duties that require a high degree of skills/accuracy and carries out assignments with minimum supervision of the registered nurse (RN). The Licensed Practical Nurse (PACT): Ensures proper identification of patient/resident per policy before administering care. Assists co-workers willingly without having to be asked and presents an attitude that promotes unified teamwork. Functions, in collaboration, with the registered nurse in identifying needs of patients and carrying out assignments. Assists the registered nurse with the admissions of patients. Assists with orientation of new personnel. Performs duties in a safe manner to prevent patient/resident/employee accident/injury. Recognizes and reports acute changes in patient conditions to the provider and documents observations appropriately. Participates in the evaluation of nursing care through attendance and provides input at team conferences. Maintains good rapport with patient/residents, families, volunteers, visitors, and co-workers. Assures completion of assignments according to policy, procedure and plan of care, and seeks other assignments as time permits. Utilizes principles of infection control, cleanliness, and hand washing to assure a safe and clean environment to prevent cross infections. Demonstrates the ability to utilize the computer independently for obtaining patient information and maintaining accurate input of data. Recognizes and communicates with NM/team leader and/or designee the need for additional instructions for self and those under their direct supervision. Consistently engages in activities that support the mission, values, and strategic initiatives of James H. Quillen VA Medical Center (JHQVAMC).
