The Licensed Practical Nurse works collaboratively in an interdisciplinary coordinated care delivery model (i.e., patient aligned care team). The incumbent is primarily responsible for patient and clinic flow, efficiency of the clinic, customer service, and teamwork. Duties include, but are not limited to: Develops/maintains effective and efficient communication with the patient, interdisciplinary team, VA medical centers, and other agencies; Checking vitals; Processing reminders, reviewing and processing alerts appropriately; Administering medication; Participate in team huddles and meetings to manage and plan patient care; Setting priorities and deadlines; adjusting the flow and sequencing of the work to meet team and patient needs; May work with the team to reinforce the plan of care and self-help solutions and entering appropriate information into the electronic record; Monitoring pre-appointment requirements to assure readiness for patient visit/procedure (e.g., X-ray, lab work); Participate and provide input in problem solving on operational issues or procedures in team meetings; Participate in and independently following up on team huddles by sharing information and collaborating with the medical team to assure continuity of care; Evaluate patient information and clinic schedule lists to determine whether patient is vested; Educate providers about shared patients (those who receive their care at multiple VAs or those who have care in the community) and bringing to the attention of the provider; Work with Veterans and their families during times of emergent and non-emergent times; Keep Veteran informed during visit and provide needed updates, assist with walk-in patients as schedule allows; Daily cleaning and stocking of rooms; Review secure messages, FAXCOM, and CMT tasks; provide SBAR communication; Assist in scrubbing the schedule. This LPN position has the potential to cross- train to other departments in the VA to help better fulfill the mission of caring for our Veterans, and to be able to fill in for other departments during times of absences and low staffing as indicated by their supervisor/management. Additional duties or tasks may be assigned by the supervisor to ensure efficient daily operations. Per CDC regulations, the LPN will be rated for exposure to blood-borne pathogens as a moderate risk.
