This GEC Float Registered Nurse (RN) position is located in the Patient and Extended Care Service Line within the Togus VA Medical Center. This is a full-time position at 40 hours per week. Serves as a float nurse for the GEC. Provides direct patient care services to the Veterans enrolled in GEC. Demonstrates self-direction in providing nursing care to patients with complex care needs in a multidisciplinary setting for GEC, HBPC, and H&PC outpatient Veterans. Provides initial and ongoing assessment of patients to identify needs, care goals, and appropriate resources necessary to Veterans. Responsible for ensuring high quality practices in the care of acute and chronic illnesses of the elderly Veterans through the coordination and integration of various nursing, medical and complementary interventions. This responsibility includes coordination of the efforts of the multiple disciplines involved in caring for patients in either the GEC clinic or in the home setting. Provides leadership in application of the nursing process and identifies resources and critical factors for achieving desired outcomes for health maintenance/improvement for patients in which he/she serves.
