This RN provides care and counseling of persons in the promotion and maintenance of health & prevention of illness and injury. This includes systematic data gathering, assessment, appropriate nursing judgment, evaluation of human responses to actual or potential health problems, case finding, health teaching, health counseling, provision of care supportive to or restorative of life and well-being, and executing medical regimens/treatments prescribed by a licensed or legally authorized provider. · Effectively implements health promotion, prevention, and management for young adult to older adult Veterans requiring skilled nursing interventions in a Veteran home based setting. · Provides care for Veterans with complex, longitudinal, mixed chronic medical and/or psychiatric conditions, Veterans with cognitive impairments, and Veterans who are terminally ill. · Utilizes the nursing process when assessing, planning, implementing, evaluating, and documenting nursing care provided and initiates changes to ensure quality care. · Works collaboratively in coordinating with the interdisciplinary team when assessing, planning, and providing and evaluating care to Veterans at home and their families. · Functions independently as a charge nurse in the program when assigned, organizing and delegating activities required in Home Based Primary Care (HBPC) to other RNs, LPNs, and providing feedback to the Nurse Manager regarding the practice of others in order to improve care. · Complete full admissions and provide direction to HBPC team members based on his/her assessments on the needs of the HBPC Veteran. · Evaluates and analyzes care provided to Veterans to assure it is timely, appropriate, cost effective, and of high quality. · Provides home based primary care by identifying problems and areas of concern, developing and initiating a plan of action, careful documentation of all findings and recommendations, and evaluating the effectiveness of action. · Establishes methods to monitor Veterans' progress and maintains appropriate documentation. · Sees to ongoing, comprehensive evaluation and improvement of clinical practice and health care delivery systems to meet the needs of the Veteran in a home based setting. · Valid drivers license required Work Schedule: Monday-Friday, 8:00am-4:30pm Telework: Ad Hoc Virtual: This is not a virtual position.
