The LPN is a vital member of the Mental Health Primary Care Clinic. They review medications, complete vitals, clinical reminders, provide short term one-to-one care/close observation to maintain safety for suicidal/homicidal patients, perform age specific and culturally sensitive nursing care for adult and geriatric patients with a variety of physical, mental, behavioral or psychological needs to include Veterans experiencing homelessness, severe mental illness/substance use disorders. The Licensed Practical Nurse (LPN) of the Mental Health Service will provide clinical and technical support to patients. Evaluate the Veteran's condition and current status, and developing an individualized plan of care to address the particular needs of the Veteran. Prepares and administers prescribed medications and performs treatments per established policies/procedures. Administers immunizations per established policies/procedures. Assist veterans in learning self-medication management Assist veterans in learning independent health and disease management. Effectively execute walk-in management. Assists other staff to prioritize the overall veteran care needs and adjusts the plan of care as needed. Has astute assessment skills, which allow for observation of obvious and subtle changes in the veteran health condition. Responds diligently to the veteran changes and deteriorating condition in emergent situations, notifying appropriate personnel in a timely manner. Facilitate point of care labs. Assure continuing improvement of service to patients, including implementation of an effective customer service program. Interact with the family and/or significant others to facilitate Veteran's self-care and independence. Assure detailed, current, and comprehensive documentation is contained in the patient medical record and other pertinent records. Demonstrates flexibility by willing and promptly accepting changes in work assignment and locations. Work Schedule: Monday - Friday 0730-1600 Financial Disclosure Report: Not required
