The Registered Nurse for Caregiver Support Program are located in Veterans Health Administration Greater Los Angeles. The Registered Nurse is responsible for for identifying, facilitating, and coordinating patient-centered care and services. Provides leadership in the application of the nursing process to patient care, organizational processes and/or systems, improving outcomes at the program or service level in the Caregivers Support Program. The Caregiver Support RN incumbent will function in support of caregivers, Veterans and family members throughout the medical center and in community settings. The RN is responsible and accountable for the provision of direct and indirect care and for the delivery of quality nursing care to a culturally diverse Veteran population. The RN possesses the knowledge and skills to effectively apply all aspects of the nursing process with collaborative, interdisciplinary, evidence-based practice, which will enable effective care coordination and implementation of health promotion and disease prevention practices. Duties are below but not limited to: Safely and effectively uses patient care equipment including appropriate cleaning of reusable Medical Equipment(RME)per facility policy and procedures. Completes initial assessment of patient condition and response to treatment and initiates follow up as needed. Able to assess mental health/behavioral changes and complete a suicide risk assessment as needed. Recognizes overt abnormal signs and symptoms and/or changes in patient's condition and responds appropriately. Implements physician's/provider's orders in a timely and complete manner. Makes assignments and directs activities of other unit staff. Demonstrates compliance with infection control practices, such as hand washing and wearing of appropriate personal protective equipment. Recognize the patient's health problem and able to manage the problem safely and effectively, recognizing his/her scope of practice. Preferred Characteristics/Experience: At least 2 years experience in mental health setting preferred. Able to work independently as well as collaborate with multidisciplinary team. Experience in initiating and participating in process improvement/quality improvement projects. Work Schedule: Full-time. Shift will be discussed during interview.
