A. Consistently provides basic physical care for and/or assistance to patients including bathing, shaving, grooming, feeding, ambulating, weighing, positioning patients, transferring, skin care, neurogenic bowel, and bladder care. B. Measures and records vital signs and patient oral intake and urinary drainage output as assigned accurately on flow_ sheets and CPRS. Reinforces fluid restrictions. Reports to RN and deviations from normal. C. Prepares and administers prescribed medications, orally, topically and by injection. D. Assists patients with enteral feedings as assigned, e.g., gastrostomy, jejunostomy, and consistently implements aspiration precautions. E.
