This position is located at the Wilmington VA Medical Center in the community living center. Adhere and promote desired outcomes for patients in the CLC in accordance with prescribed guidelines. Provide, direct and supervise the delivery of quality nursing care to a culturally divers veteran population. Promote and maintain safe comfortable and therapeutic environment. Perform systematic assessments of patients and assure that data is collected, documented and communicated. Assess patient and family understanding of therapy regimen and conduct ongoing education for patients and their families treatment requirements. Responsible for implementation and administration, monitoring and documentation of patients response to prescribed treatments, including appropriate notification of adverse reactions to physicians orders are transcribed and entered into the medical record in a timely manner. Conduct all tasks necessary for management and care of a patient in the Community Living Center environment including but not limited to wound care, medication administration, completion of Minimum Data Set (MDS), monthly nurses notes and other duties as assigned. Monitor the environment for potential and/or actual safety hazards. Direct supervision of licensed and unlicensed staff.
