The LPN requires a general review of their work by an RN. The LPN demonstrates collaboration with Core Teams (Patient, Primary Care Provider, RN Care Manager and MSA and the Expanded Team (family/caregiver, internal and community-based services involved in providing care to the patient), Phlebotomy, Telehealth and Ancillary services. The LPN is required to float to Bath Primary Care Clinics, Specialty Care Clinics, CBOCs, and Rural Health Clinics as assigned. Basic Requirements: Citizenship. Citizen of the United States.
