HBPC PACT Case Manager: Reporting directly to the Nurse Manager/Program Director, the HBPC Care Manager is accountable for adhering to established policies, standards of care and practice. They serve as facility champion and coordinates local HBPC PACT implementation, training, communication, outreach and sustainability. The case manager plans, organizes, directs and evaluates the patient on the HBPC PACT team and reports related actions/activities to Program Director and Medical Director Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
