The Cardiology Nurse Coordinator is responsible and accountable for all elements of the nursing process when providing direct patient care. The Registered Nurse assumes responsibility for the coordination of care which includes the transition through the continuum of care, patient and family education, patient self-management after discharge, follow-up care and supporting factors that impact customer satisfaction. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: Cardiology-ICU-Telemetry experience within the last year - Three (3) years of nursing experience, and BLS/ACLS current or upon hire. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience. Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required. Physical Requirements: The position involves considerable standing, walking, stooping, bending, reaching, lifting, and turning. It requires restraining confused patients, repositioning patients, assisting with transferring patients to wheelchairs/stretchers/cars, and transporting them to other department or facilities. Mechanical devices are available and are required to be used for heavy or difficult to lift patients. The work may involve pushing or lifting weight in excess of 50 pounds and going up and down flights of stairs. The Registered Nurse may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. This position requires visual acuity, good hearing, distinctive speech and manual dexterity. The position requires the nurse to work for periods of time with arms above shoulder level with neck in a fully flexed or extended position and the ability to physically control or protect his/herself from physically abusive patients. They can perform nursing functions and the duties as assigned independently and will alter the daily duties to meet the population and/or service needs. The Registered Nurse assists with patient care needs throughout the Cardiology section including patient preparation, moderate sedation and other needs during procedures, and other duties as assigned. VA offers a comprehensive total rewards package: VA Nurse Total Rewards MAJOR DUTIES: Use the nursing process to asses, plan, implement care and evaluate patients Provide patient/family education needed for a procedure, including education on the procedure to be performed, pre-procedure instructions for prep and post procedural care. Collaborate with the Interventional team to ensure the correct procedure is scheduled and any special considerations or equipment is needed. Work collaboratively with the Cardiology providers by assisting with medication titration and adjustments to prevent additional ED and/or clinic visits, ensure patients complete recommended testing and receive testing results, and provide follow-up telephone calls to asses blood pressure and lab results. Assist in ensuring encounters are complete within 7 days of the encounter to ensure accurate workload is documented. Serve as coordinator during the evaluation process, and collaborate with other service team members to manage the care of patients being followed by the Cardiology Clinic, which includes general Cardiology, Congenital Heart, and Cardiology Procedures Identify potential emergency/crisis situations and provide appropriate interventions Ensure patients are scheduled appropriately and timely for cardiology clinic and procedure appointments Ensure patient records from outside providers and hospitals have been obtained prior to clinic appointments and/or procedures and send records to be scanned into the patient medical record. Monitor the general Cardiology clinics for clinic utilization and missed opportunities to ensure access to clinic can be met. They assist with adjusting clinics to meet patient care needs such as obtaining approval for overbooks or identifying alternative creative measures to meet clinic needs. Assist with patient triage whether in person or via telephone to ensure their concerns and/or symptoms are addressed. Use critical thinking/judgement skills to identify potential emergency/crisis situations and provide appropriate interventions. Triage all incoming Cardiology consults for cath, PCI, clinic, etc. Manage alerts in CPRS daily Seek out and participate on committees and task forces as needed/appropriate. Actively participate in unit/program level quality improvement processes and initiatives, customer service programs, and involvement with research Coordinate plan of care with appropriate follow up care coordination in collaboration with provider for inpatient and outpatient Cardiology patients, especially the transition from inpatient to outpatient. Facilitate maintenance of care by contacting patients who miss appointments or who are not adherent to medication regimens, or scheduled follow-up procedures.
