The Outpatient Diabetes Educator – Registered Nurse, under general guidance, is responsible for providing age-specific diabetes self-management education to patients and healthcare professionals. The Educator provides diabetes education services in a new core delivery model as a therapeutic lifestyle coach for corporate organizations and their employees. Adheres to the AdventHealth Corporate Compliance Plan and to all rules and regulations of applicable local, state and federal agencies and regulatory bodies. Actively participates in outstanding customer service and accepts responsibility for maintaining relationships that are equally respectful to all.
The value you’ll bring to the team:
Provides diabetes instruction in group and individual sessions, in accordance with patient education standards set by the American Diabetes Association and departmental policy. Provides education in all content areas, except individualized meal plan calculation. Provides day, evening and weekend education programs as requested at any AdventHealth Campus. Able to triage and problem solve unexpected situations that interfere with the delivery of an optimal education experience for the patient.
Obtains individual patient assessment. Identifies patient/family problems or learning barriers including physical, cognitive, environmental, language/cultural, financial, psychosocial and spiritual and provides appropriate referral as necessary. Establishes individualized learning goals, objectives and educational plan in collaboration with the patient and the patient’s caregiver(s). Evaluates patient upon completion of educational plan and provides remediation/plan modification as indicated and in consultation with diabetes team members as needed. Provides patient follow-up by telephone, mail, or e-mail as indicated and according to established department guidelines.
Provides ongoing therapeutic lifestyle coaching utilizing only approved resources to effectively engage the client. Manages assigned case load in order to complete calls in a timely manner according to call schedule, as well as complete all documentation and correspondence within the timeframe provided by the project manager. Reports any clinical significant barriers to project manager in a timely manner.
Participates in practice changes as a result of performance improvement activities. Utilizes accurate documentation and monitoring of activities as needed. Supports quality standards and initiatives set by department. Educates patients and family about current areas of research within AdventHealth Diabetes Institute and the Translational Research Institute. Assists with identification of potential candidates for research studies as requested.
Serves as an advocate for care with community agencies including pharmacies, insurance companies, and employers. Collaborates with other team members to address patient and family needs. Participates in team meetings for education and discussion of patient care issues. Complies with the American Diabetes Association Education Recognition Program requirements for continuing education until CDE credential is obtained. Seeks professional growth opportunities to advance care and knowledge related to diabetes. Participates in community activities as requested, such as, presenting educational lectures and assisting with support groups for the community in general.
Qualifications
The expertise and experiences you’ll need to succeed:
Minimum qualifications:
Associate of Science degree in Nursing
Current valid State of Florida or multi-state license as a Registered Nurse
One (1) year of experience in field
One (1) year of experience in diabetes specialty
Certified Diabetes Educator (CDE) by the National Certification Board of Diabetes Educators (NCBDE)
Basic Life Support (BLS)
Advanced knowledge of diabetes concepts and skills including insulin pump therapy
Ability to provide comprehensive insulin pump training and glucose sensor training
Ability to provide teaching in all aspects of diabetes management
Preferred qualifications:
Bachelor of Science in Nursing
Two (2) or more years of experience in the field
Bilingual in Spanish
Knowledge of continuous glucose monitoring
