The role you’ll contribute:

The Special Pediatric-to-Adult Need (SPAN Coordinator) under the direction of the Clinical Manager/SPAN Director (physician), provides clinical/triage support for SPAN patients as directed by the Physician. The SPAN Coordinator schedules and performs pre-visit assessments for new SPAN patients. The SPAN Coordinator triages patient calls, refill prescriptions according to refill protocol, and accurately inputs patient cases in EMR. SPAN Coordinator must possess computer skill set to appropriately document pertinent patient information in EMR in accordance to office protocols and guidelines established by practice. The SPAN Coordinator functions as a resource to providers/patients. Customer service skillset essential in all aspects of role.

 

The value you’ll bring to the team:

·       Assist with scheduling SPAN patients utilizing appropriate schedule modalities

·       Perform pre-visit assessment on all new SPAN patients (when applicable)

·       During intake assessment, obtain/document in EMR - ROI, Family History, Social History, Past Medical History & reason for visit

·       Ensure prior patient medical records are available prior to scheduled appointment

·       Order psychological testing (if applicable) as directed by provider

·       Coordinate Home Health Care and Care Management as directed by provider

·       Assist with patient referral as directed by provider

Qualifications - External

The expertise and experiences you’ll need to succeed:

Licensed Practical Nurse (Kansas)

               BCLS obtained through the American Heart Association
