Responsible for preparing the patient for an upcoming office through advanced outreach. Provides support to providers by collecting screening data from the patient in advance of the appointment. Identifies patient care opportunities and educates patient on the importance of completing recommended preventative health screenings. Provides high quality scheduling experience for the designated patient population as needed. Deploys excellent customer service engagement skills by phone.
High School Diploma or equivalent required
Licensed to practice as an LPN in the Commonwealth of Pennsylvania required
Two years of Health Care Experience required
KNOWLEDGE, SKILLS, & ABILITIES:
Knowledge of ambulatory care flow, quality, and working with physicians
Proficient use of Microsoft Office, including extensive use of Excel
Excellent clinical skills, teaching skills, interpersonal skills, analytically minded, able to learn systems quickly
Ability to work independently as well as within a team
Ability to multi-task, prioritze workload, and effectively manage competing priorities
Analysis, synthesis, and problem solving skills
Deductive and inductive thinking skills
Previous use/exposure to using an EHR/EMR, working with clinical data, clinical reporting and other analytics tools preferred
